Each stage has a purpose, questions it must answer, activities it performs, an output it produces, and a business value it creates. Nothing in the next stage is legitimate until the previous stage's output exists.

Not how it is described in documents — how work actually moves through it. We map the real system: the people, the handoffs, the information paths, the controls, and the places where performance is being lost.
Why Most Transformations Fail
Transformation programmes rarely fail for lack of budget or software. They fail by inverting the method — by starting at the third stage and assuming the first two were unnecessary.
Software is selected, configured and launched on top of the existing process. The process is inherited intact — now with a login screen. The organization paid for the change it didn't make.
The future state is drawn to match the current structure, or an aspirational one that was never costed. The real process — with its workarounds, handoffs and unwritten rules — is never examined.
The system goes live; the way of working does not. Familiarity wins. Within a quarter the old process runs on top of the new system, and the investment is declared a failure.
The project closes, the vendor leaves, and the new way of working decays back toward the old one — because nothing was designed to keep it improving.
